[annotator-dev] An example of an annotator.js plugin?

Hi all,

I want to write a plugin that does the following:

1) Hides the text entry.
2) Shows a set of radio buttons, and the user can select one of the 
available categories.

There used to exist a plugin that did that, but it has stopped working.
In the documentation I couldn't find any example on how to write such a 
plugin.
