[ckan-dev] Move to Git

I've done some more changes for the Github move:
* added a tag for the ckan-1.5 release (all the tags seemed to have got
lost in the move)
* configured github and readthedocs to automatically compile the docs
* updated: http://docs.ckan.org/docs/ckan/en/latest/install-from-source.html<http://readthedocs.org/docs/ckan/en/latest/install-from-source.html>
* updated http://wiki.ckan.org/Submitting_a_code_patch

There are still a few wiki pages to go, if anyone has a chance:
