[foundation-board] Barclays identification for Board Members

Dear Board,

Apparently we need to identify some good portion of the board to Barclays
due to money-laundering regulations.

Attached in the relevant form. It would be good to agree tomorrow which
Board members would volunteer to fill this in and identify themselves to
Barclays at their local branch.

I apologise for this hassle - an unfortunate consequence of ever stricter
regulations for banks.
