[Open-access] Open Access Initiative - meeting notes, names and moving forwards

I've set up http://wiki.okfn.org/Working_Groups/access (if I'm write in
thinking @ is not an allowable URL character in that context).
We can change the URL ending if you prefer. Those without one might need to
get an okfn wiki login, which is a very quick process.
