[Open-data-census] Census Revision error

hi all
I have try to make revision at Senegal open data census

http://census.okfn.org/country/overview/Senegal

I am already login and I click the the bouton "Propose Revisions" at the
same line to "Company Register" and I got this page

http://census.okfn.org/country/submit/?dataset=companies&place=Senegal

After filling the form I got the error message
