[School-of-data] End of courses quizzes

Everyone,

We're looking for solutions of little embeddable quizzes for the end of our
courses - we've experimented with google docs for our what is data quiz:

http://schoolofdata.org/handbook/courses/what-is-data/

while the form looks nice - it doesn't offer easy feedback for users and
allow us to measure the number of people taking the quiz and successfully
completing the quiz.

Is there any solution you know of that works well? You have previously
worked with?
