Specifically, is this the case, today, for NIH funded scientists?  Are
they forced to pay publishing fees by journals in exchange for obeying
the NIH mandate?  And are they having to pay these fees out of their
own pockets?  Thus costing them tens of thousands of dollars?

Can someone with first-hand experience please chip in here.
