I incorporated this on the wiki page. Ideally, each of the points should
become a heading for a collection of real-world examples and best
practices. Should we start a series of blog articles/interviews to speed
up the writing of such a collection?

Agree - see the comment on ICAZ by Eric. One thing I'd like to
investigate more is what's the right mix of bottom-up and top-down
action, in terms of maximizing involvement and participation of
community members.
