[foundation-board] Directors and Officers insurance

Following the last board meeting, there was a request to review what D&O
insurance might offer us.  Quotation summary and policy document attached
from AXA, which at £1350+insurance premium tax (6%) is the best quote we've
had thus far.

Do let me know if you have any questions; this can be discussed again at
the next board meeting.
