Thanks Jane, this is helpful for keeping us grounded. I agree that a culture change is required, as others' messages have also pointed to. 
Having said this, I've very much enjoyed the discussion around data quality (thanks in particular to Dave S). If ever there was an elephant in the room this is it. At the high level, I'd add that we also need to incentivise better data collection processes and cultures. It won't just happen though good will when staff are stretched for time and/or coding tasks get left to the lowest paid workers.
Happy to comment on the high level draft if appropriate.
